One-on-one sessions by video. Assessment, hands-off treatment, and a program you carry into your week — from home, work, or wherever you are.
In-home visits
I come to you across northern New Jersey — Bergen, Passaic, and Morris counties — and southern New York, including Rockland and Orange counties. Hands-on care and balance work in the space where you actually move every day.

Who I am
Hi (Cześć), my name is Claudia (Klaudia) Stopka, PT, DPT!
I'm a Doctor of Physical Therapy specializing in vestibular rehabilitation, generalized orthopedic neck and back pain.
I created Nova Forma Physical Therapy to offer all individuals to feel heard, understood and supported throughout their therapy journey.
I became a physical therapist because I believe movement has the power to change lives. Throughout my career, I've seen how pain, dizziness, and balance problems can affect every aspect of a person's daily life—from working and exercising to spending time with family.
As healthcare continues to evolve, I believe physical therapy should evolve with it. People deserve expert care that fits into their lives, not the other way around. That's why I offer both virtual and in-home physical therapy, making personalized treatment more convenient, accessible, and effective.
My goal is simple: to help every patient move with confidence, improve their balance, regain their independence, and return to the activities they enjoy.
As a bilingual physical therapist fluent in both English and Polish, I'm proud to provide care that helps patients feel understood, comfortable, and supported throughout their rehabilitation journey.
